1. Data controller

Under Turkish Personal Data Protection Law No. 6698 ("KVKK"), your personal data is processed by Dentia Oral and Dental Health Clinic (the "Clinic") as data controller, within the framework described below.

2. Personal data processed

Through the online booking form and our contact channels, the following data may be processed:

• Identity: first and last name
• Contact: mobile phone number or email address
• Appointment: chosen doctor, treatment, date and time
• Health information: complaints or explanations you voluntarily write as an appointment note (special category personal data)
• Transaction security: technical server logs kept to keep the site secure

3. Purposes of processing

• Creating, confirming and managing your appointment
• Communicating with you about your appointment and treatment
• Planning and delivering the healthcare service
• Fulfilling legal obligations
• Keeping the site secure and working correctly

4. Legal grounds

Your personal data is processed on the grounds that it is directly related to the conclusion or performance of a contract, that it is necessary to fulfil a legal obligation, and for the legitimate interests of the data controller (KVKK Art. 5/2).

Special category data such as health information is processed under KVKK Art. 6 on the basis of your explicit consent or the other conditions set out in law. The consent box on the booking form is used for this purpose.

5. Transfer of data

Your personal data may be transferred, strictly limited to the purposes above, to technical infrastructure providers needed to deliver the service (such as hosting, calendar and messaging services) and to public bodies authorised by law.

If the providers' servers are located abroad, the transfer is carried out in line with KVKK Art. 9.

6. Retention period

Your personal data is kept for as long as the purpose of processing requires and for the periods set out in the relevant legislation. When that period ends, it is deleted, destroyed or anonymised.

7. Your rights

Under KVKK Art. 11 you may apply to the Clinic to exercise the following rights:

• To learn whether your personal data is processed
• To request information if it has been processed
• To learn the purpose of processing and whether it is used accordingly
• To know the third parties to whom it is transferred in Turkey or abroad
• To request correction if it is incomplete or inaccurate
• To request deletion or destruction within the conditions set out in law
• To request that corrections and deletions be notified to third parties to whom data was transferred
• To object to a result against you arising from analysis of the data exclusively by automated systems
• To claim compensation if you suffer damage because of unlawful processing

8. How to apply

You can send requests about your rights by email to randevu@dentiapoliklinik.example or in writing to the address above. Your application is concluded within thirty days at the latest, depending on the nature of the request.
